Primary Home Care (PHC) is a non-technical, medically related personal care service prescribed by a physician as part of a client’s plan of care. PHC is available to eligible clients whose health problems cause them to be functionally limited in performing activities of daily living. Patient service is provided by a supervised Personal Care Attendant, who performs tasks based on individual need.

The Personal Care Attendant has been trained by a licensed nurse to perform the following duties:

• Changing bed linens
  
• Light Housecleaning
  
• Dressing and Laundry
  
• Shopping and Escort Assistance
  
• Meal Preparation, Feeding and Dish Washing
  
• Exercising
  
• Assist with Self-Administered Medication
  
• Toileting
  
• Transferring/Ambulation
  
• Routine Hair and Skin Care
